We help individuals and families prepare and plan for practical & emotional issues in aging, changing health or incapacity. The best decisions are made in a time of relative stability and calmness. We work with families who are willing to explore and plan ahead to give them future choice and control in health, financial and legal decision making.

PHCA clients are individuals or families who want to have control over current or future health care decisions. Our client families may be:

  • Living with a medical crisis and looking for options, both public and provide
  • A proactive adult who wants comprehensive planning (health, financial and legal) in place in case of sudden illness to reduce family burden in crisis
  • Parent(s) with young children wanting to have plans in place for sudden or critical illness or death. Health, financial and legal (including guardianship) considerations are reviewed.
  • Experiencing increasing concerns around parent health, safety or coping at home
  • Seeing cognitive or personality changes in Aging Parents impacting safety /independence
  • In conflict with siblings over how best to support aging parents
  • In the Sandwich generation stressed by parent care issues that impact their work, family or even their own physical or mental health
  • Experiencing frustrations or questions with how to navigate the health care system
  • Struggling with housing options, Aging in Place or relocating to receive needed services with reduced responsibilities.
  • Looking for quality, accountable community resources
  • Unaware of Aging Parents’ health, financial or legal wishes or plans and need direction
  • Managing chronic or end-of-life illness and need support and guidance to understand what help is available and how to plan for the future
  • Experiencing Caregiver stress or burnout.
